Before proceeding to the online forms: Parents/Guardians and students must read the Policies and Information sections

a. Click the “My Student” tab at the top to view the Policy and Information tabs and/or to print a Physical and Health History form

b. Click the “Physical” tab if you need to print the Physical and Health History forms

c. Click the Policy and Information tab to view all of the policies and information

d. After reading all of the policies and information click the “add a child” link to continue

e. Link your student (You must use his/her Legal First and Last Names)

f. Click “Link my Student”

Complete the following online forms and submit:

a. OSSAA Student Eligibility form
b. Athletics and Activities Consent and Signature page

All fields must be completed before the forms can be submitted.
